New Team Member Paperwork

Completing your electronic new hire paperwork, commonly called your onboarding forms, is the first step in becoming a member of the Messiah Lifeways team. Onboarding forms should be completed prior to your pre-employment physical.

Onboarding consists of 7 forms that, once completed, are submitted electronically. After you receive and review your offer letter, please complete your Onboarding forms immediately. Click here to get started.

Important Reminders:

It is required that you use Internet Explorer as your browser.
Be sure that your pop-up blockers have been turned off—you want to allow pop-ups.

Instructions on how to turn off pop-up blockers:

  • Click the "Tools" drop down menu

  • Click "internet Options" at the bottom of the menu

  • Go to the "Privacy" tab

  • Uncheck the box next to "Turn on Pop-Up Blockers"

  • Click Apply, Save, OK

Mozilla Firefox, Google Chrome and Safari will not allow you to submit the forms to us electronically.
Mobile devices and tablets are also not recommended or supported.
Make sure to use the most current version of Adobe Reader. 

Once you complete the onboarding forms, you will also be scheduled to come to Human Resources to receive some additional paperwork. This meeting will take approximately 30 minutes. During this time you will have your photo taken. You will also need to bring: 2 forms of identification (click here to see the list of acceptable identification), any certification/license needed for your specific job (must be the wall copy), proof of high school graduation (Enhanced Living team members only), and work permit if you are under age 18.
